Reporting lost and stolen cards and fraudulent activity

PC 9.1 - Reporting Lost or Stolen Cards

If a card is lost or stolen, the cardholder or monitor must immediately notify BOA by calling (866) 500-8262.

The cardholder or monitor is required to report the stolen card to the appropriate local law enforcement agency. In unincorporated Hillsborough County, call the Sheriff at (813) 247-8200 or the City of Tampa, call (813) 231-6130 and give details to the Emergency Complaint Writer. If a card is stolen within a municipality, call the police department of that municipality. The Cardholder or Monitor is required to obtain a Report Number from the law enforcement agency.

Note: Replacement cards are generally issued within two (2) business days of the request.

PC 9.2 - Reporting Fraudulent Activity

If a card account has fraudulent activity, the cardholder, monitor or purchaser must immediately notify BOA by calling (866) 500-8262. Cardholder may be required to complete an Affidavit of Fraud.

When BOA contacts the Cardholder, the Cardholder must follow the instructions and if fraudulent activity occurred, immediately email PCA at PCardAdmin@HCFL.gov with findings. The contents of the email must include those listed under section PC 9.4.

PC 9.3 - Vendor Notification of Lost, Stolen, or Canceled Cards

The cardholder shall contact any vendor who has their card account number on file (e.g., subscriptions, mail order, telephone order) to ensure that the vendor updates the records.

PC 9.4 - Reporting Lost, Stolen, or Closed Cards for Fraudulent Activity

Cardholders, monitors, purchasers, or coordinators must email PCA at PCardAdmin@HCFL.gov within one (1) business day of discovering a lost or stolen card or fraudulent activity. The email should include the card account name, department name, date and location of loss, law enforcement report date (if applicable), notification date and time to BOA, and a list of purchases made on the day of the loss or theft.
